Thank you for choosing to have your hockey organization take Jack's Pledge!

All Minnesota Youth Associations, high school team and non-youth teams across the state, as well as Youth Assocations and teams outside Minnesota please register with Jack's Pledge by clicking the Continue button below and completing the online form.

After you register here, please execute the Player and Coach pledges within your ogranization.  Please print the pledges (PDFs below) and have your members sign them. We'll send you helmet stickers to show your support. We'll also list your hockey organization by name in our Who's Taken The Pledge section of this site. Please consider photographing your pledge signing events and sending them to us to share with the Jack's Pledge Community.
